Transition

Easing the emotional and practical journey, for children and families, as they transition to and from Sunfield.

Sunfield’s dedicated Transitions Coordinator works alongside the Referrals and Admissions team to support the planning and coordination of all new student admissions into Sunfield, and also oversees the eventual transition of students from Sunfield into adult services.

Transition to Sunfield is tailored to your child’s needs, and involves the expertise and input of many of our teams, who will work closely with your family to ensure your child has a smooth transition. Transition from Sunfield

We have been supporting student transitions for a decade and have vast experience and strong long-term alliances within adult service providers nationwide. This enables us to confidently support a smooth transition tailored to each young person’s needs, interests and ambitions, safe in the knowledge that they will continue to receive the same high level of support they have enjoyed at Sunfield.

When your child is ready to transition from Sunfield, we will work with them, your family and any related professionals, agencies and local authorities, to develop a bespoke Transition Plan. We will help you to understand the relevant legislation, policies and procedures and ensure your voice is heard during the planning process. We will also assist with the adult assessment process, and provide a Transition Profile of your child, to offer a true reflection of their needs, so the right provision can be found. We will also coordinate the timely implementation of their Transition Plan.

Life after Sunfield

Most students move from Sunfield into residential or supported living environments, where they continue to be cared for 24/7, and can follow individualised programmes of leisure and learning. Some attend further education colleges, on either a full or part time basis, while others access work-related learning or a work experience placement. For some students, a future programme may consist solely of leisure and life skills activities. Whichever direction they take when they leave Sunfield, we aim to ensure that your child will leave with the confidence to communicate and contribute to the community they will be joining.
